OUR SERVICES
All services offered through the Assured Development Services program will be structured to promote “person centered” measures which promotes the involvement of the individual and their family members in the decision-making that is relative to their placement and overall care. Each individual admitted will receive an assessment and supported in the development of their Individualized Service Plan (ISP), which will guide the scope of their services.

Extensive Assessments at the beginning of services, annually and as needed by qualified individuals.

Training and Skill Development on a daily basis according to the individual’s goals and objectives.

Services that are guided according to the “Person Centered” approach of inclusion relative to decision making and identification of personal preferences based upon the concept of “what’s important for the individual and what’s important to the individual.”

Promotion of Self Sufficiency including services and training supports designed to increase each individual’s ability to complete tasks with greater independence and involvement. These procedures will increase self-pride, personal knowledge, skill development and independence.

The promotion of a Family-Like Atmosphere designed to promote activities which strengthen the connectedness between Assured Development Services staff, the individual, their family members and authorized representatives. (i.e. holiday celebrations, family dinners, birthdays, etc.)

Individualized Treatment Planning developed by qualified Assured Development Services staff (QDDP designation) with input from the individual, their family members and/or authorized representatives to ensure accuracy of goals and objectives.

Case Management by a Qualified Developmental Disability Professional (QDDP) The QDDP will meet weekly with each individual to include coordination of services based upon individual needs, preferences and quality assurance. (Scheduling of medical/wellness appointments, communication with parents, family members, Support Coordinators and other providers supporting individuals in the program)

Weekly Recreational Activities to include community-based and in-house activities such as arts & crafts, cooking exploration, horticulture, money management exercises, weekly trips to local retail stores, recreational outlets with cultural, educational and historical significance, etc. All activities will be guided by individuals preferences and needs.

Community Engagement that provides individualized supports that help individuals acquire, maintain, or improve the skills needed to participate in community life, build meaningful relationships, increase independence, and make personal choices. Services are provided in integrated community settings and may include community education, volunteer opportunities, recreation, social activities, and skill-building that promotes inclusion with people who do not have disabilities.
