ADMISSIONS CRITERIA

Individual must be at least 18 years of age

Individual must have a primary diagnosis of intellectual disability that may also include a concurrent psychiatric condition

Individual must have an established need for support and have limitations in adaptive skills as determined by the current VIDES assessment in two or more of the following areas:
 

• Socialization

• Personal Care

• Communication

• Money Management

• Community Integration

• Use of Community Resources

• Health and Safety

• Mobility

• Functional Academics

• Behavior Management
   

Are eligible for Community Living Waiver through a community services board or private funding

Have the ability to benefit from services provided

Have clinical and medical needs that can be accommodated by Assured Development Services, LLC personnel and/or contractors

Have a current physical examination

Be free from communicable diseases (ex. Tuberculosis)

Have all admissions paperwork and physicals completed
